First Visit
Your initial appointment will consist of a consultation in which you will learn more about your diagnosis and the preliminary treatment options available to you. We may take an impression at this initial appointment in order to assist with the final planning of your treatment. During your second appointment, we will review the treatment plan and sequence, your treatment fee, and the treatment letter with you.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any x-rays if applicable.
- A list of medications you are currently taking.
- If you have dental insurance, bring the necessary insurance card. This will save time and allow us to help you process any claims.
IMPORTANT: At the time of consultation, all patients under the age of 18 must be accompanied by a parent or guardian.
It is important to inform our office if you have any medical conditions that may cause concern prior to a dental procedure (such as diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (such as heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your general dentist or specialist has taken x-rays, you may request that they forward them to our office electronically. Please pick up your x-rays if they are not digital and bring them to our office for your consultation. In the event that additional films are required, they can be taken in our facility.
